Supervisor Suggested Referral
to Duke EAP
When would you make a Suggested Referral to Duke EAP?
At the point you first recognize your employee is exhibiting work performance and/or work behavioral issues, we encourage you to refer the employee to Duke EAP.
Why make a suggested referral to Duke EAP?
Many employees may be unaware that EAP is available whenever they or their family need help with a concern. Managers play a key role in alerting employees to this valuable resource. By suggesting Duke EAP to your employee, you are offering assistance that could improve your employee's work performance and/or behavior.
The following steps will guide you through the process of making a Suggested Referral to Duke EAP.
Step 1: Identify for yourself the job performance and/or behavioral concern/s (Consult the Job Deficiency Checklist for a listing of potential problem areas).
- Be specific and factual regarding the employee's behavior and/or performance;
- Do not attempt to diagnose the issue or concern;
- Specify and document your expectations for the employee's improved job performance and/or behavior.
Step 2: Contact your Human Resources Consultant and/or Duke EAP (1-800-336-3853) for coaching assistance in presenting the concern to your employee.
Step 3: Meet with the employee to:
- Discuss documented job performance and/or behavioral issues;
- Provide the employee with a written copy of job performance and/or behavioral concerns and expectations;
- Suggest the employee seek assistance through Duke EAP (919-286-1244 or 1-800-336-3853) related to the job performance and/or behaviors outlined, and;
- Schedule a follow-up meeting, within a reasonable time frame, to review the employee's job performance and/or behavioral improvement.
Step 4: If the employee's job performance and/or behavioral concerns have improved by the next scheduled meeting, provide both written and verbal communication of improvement to the employee and continue monitoring job performance and behavior.
If the employee's job performance and/or behavioral concerns have not improved or have worsened by the next scheduled meeting, follow the steps outlined for the supervisory referral. In addition, consult your Human Resources Department to ascertain your company's policies and disciplinary process.
